‘Limited’ employee W-2 forms lost at Palos Community Hospital

Palos Community Hospital has lost a “limited” number of its employees’ W-2 forms and is trying to figure out “where the breakdown occurred,” a hospital spokeswoman said.
The hospital has reprinted W-2s for employees who notified the staff, but is still trying to determine how many employees are affected, spokeswoman Lori Mazeika said.
“What we do know is that the number is limited as most employees have received their statements,” Mazeika said.
“We are looking internally and externally to learn where the breakdown may have occurred. Our number one priority is the protection of any personal information of our employees,” she said. “We have measures in place to address any issues that may arise, and we are ready to implement those measures should it become necessary.”
